Duties
- Provide direct patient care by assisting with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts.
- Monitor and record vital signs, observe changes in residents’ conditions, and communicate findings promptly to nursing staff.
- Support medication administration under supervision, ensuring accurate documentation in electronic health record (EHR) systems while adhering to HIPAA regulations.
- Assist with specialized care needs including Alzheimer’s and dementia care, memory care programs, and behavioral health support.
- Maintain a safe environment by following aseptic techniques during procedures like catheter care and infection control protocols for infectious disease care.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans for residents with complex needs such as post-acute, sub-acute, ICU, or emergency medicine cases.
- Document all patient interactions thoroughly in medical records using EMR systems like Cerner or eClinicalWorks to ensure accurate medical documentation.
- Support residents’ emotional well-being through compassionate caregiving and engaging in activities tailored to their age group or disabilities, including pediatrics or working with individuals with developmental disabilities.
Qualifications
- Valid CNA certification required; previous experience in long-term care, assisted living, nursing homes, hospitals (including Level I Trauma Center or Level II Trauma Center), or acute care settings highly preferred.
- Demonstrated proficiency with EMR/EHR systems such as Cerner or eClinicalWorks; familiarity with medical terminology and physiology knowledge is advantageous.
- Experience working with diverse populations including seniors, individuals with dementia or Alzheimer’s disease, behavioral health needs, or disabilities.
- Knowledge of infection control practices including aseptic technique and basic life support (BLS) certification.
- Ability to perform physical tasks such as lifting patients with Hoyer lifts and standing for extended periods; strong attention to detail in medical documentation.
- Excellent communication skills for effective collaboration within multidisciplinary teams across outpatient clinics, inpatient units, home & community care settings or hospice environments.
