ALF Care Coordinator RESPONSIBILITIES:
§ Makes rounds to evaluate status of residents and determine resource needs
§ Obtains assistance for emergencies and assists as necessary.
§ Identifies, reports, and documents incidents and accidents; participates in investigation of incidents/accidents as necessary.
§ Assures that caregiving practices are consistent with regulatory requirements.
§ Participates on committees as necessary.
§ Completes records and administrative reports as needed.
§ Maintains regular, reliable, and consistent attendance at the worksite as directed by supervisor.
§ Performs other duties as assigned.

§ Must be able to read, write, speak, and understand the English language.
§ Must possess the ability to deal tactfully with personnel, residents, and visitors.
§ Ability to work harmoniously with other personnel.
