A DAY IN THE LIFE OF A SOCIAL SERVICES DESIGNEE
As a Social Services Designee, you play a vital role in supporting residents’ emotional, social, and behavioral well-being. Each day, you help ensure residents feel heard, supported, and connected.
Your daily responsibilities include:
- Conducting psychosocial assessments to evaluate resident emotional and social needs
- Providing emotional support, active listening, and crisis intervention when needed
- Advocating for resident rights, preferences, and person-centered care
- Developing and updating individualized service plans focused on mental health and social needs
- Coordinating with families, guardians, mental health providers, and outside agencies
- Monitoring changes in resident mood, behavior, and functioning
- Supporting behavioral interventions and care planning initiatives
- Assisting with admissions, transitions, and discharge planning to ensure continuity of care
- Maintaining accurate documentation in compliance with facility policies and state regulations
- Must be Licensed Social Worker in the state of MN.

Pay: $52,000.00 - $60,000.00 per year
