Basin Dream Center Youth Advocate Case ManagerStaff Job Description
The Youth Advocate Case Manager monitors the day-to-day living of each resident who resides in the home, or off campus and helps them reach their full potential, through goal setting and achievements. In addition, the Youth Advocate assists the Chief Programs Officer in all aspects of program delivery.
The Youth Advocate Case Manager must be flexible in fulfilling responsibilities and day-to-day activities. This job description is intended to describe the general content of and requirements for the performance of this position. This is a full-time, exempt position.Reports to: Chief Program OfficerJob Responsibilities
· Perform checks and inventory of personal belongings during intake of new residents.
· Complete required documentation, including intake forms, goal sheets, and disciplinary forms for assigned residents.
· Provide daily on-site support to residents as necessary in collaboration with the Chief Programs Officer.
· Assist residents in setting and accomplishing weekly and monthly program goals by setting schedules, routines, coordinating services, and arranging resources.
· Provide transportation for residents to school, therapy, work, and doctors’ appointments.
· Assist residents with securing all documents such as ID, birth certificate, social security card, and driver’s license.
· Assist residents with enrollment into high school, certificate program or college.
· Assist residents with job searches, resume writing, and securing part time or full-time employment.
· Assist residents with applying for food stamps, Medicaid and/or pink card.
· Teach life skills classes when necessary to their residents who miss due to work schedules. This duty to teach will rotate between Youth Advocate and partnering organizations.
· Report residents’ progress notes to the Chief Programs Officer daily.
· Monitor residents by verifying school attendance and grades, responsible for all communication with the school, advocating for needed services and entitlements, and obtaining additional resources.
· Support Chief Programs Officer in preparation of residents’ discharge by reviewing and amplifying discharge plans, coordinating discharge and post-discharge requirements, and providing resources.
· Provide crisis intervention and de-escalation services to residents when necessary.
· Assist residents and the Youth Engagement Specialist in preparing homes for tours and open houses.
· Participate in fundraising events assigned by the Chief Program Officer.

Pay: $20.00 - $24.00 per hour
