Responsibilities
- Deliver comprehensive nursing assessments and develop individualized care plans for patients in their home environment.
- Administer medications, including injections and IV infusions, following strict protocols and ensuring patient safety.
- Monitor vital signs, perform physical examinations, and evaluate patient responses to treatment interventions.
- Manage complex cases such as dementia care, Alzheimer's care, pain management, and working with individuals with developmental disabilities.
- Utilize electronic health record (EHR) systems like Epic or Cerner to document patient information accurately and maintain medical records in compliance with HIPAA regulations.
- Provide education and support to patients and their families on medication management, disease processes, and health coaching strategies.
- Coordinate discharge planning, case management activities, and collaborate with physicians for diagnostic evaluation or urgent care needs.
Qualifications
- Valid LPN license with current state licensure required.
- Critical care experience or ICU experience highly preferred; hospital or emergency medicine background is advantageous.
- Demonstrated proficiency with EMR/EHR systems such as Epic, Cerner, or Athenahealth.
- Experience working with geriatric patients, including long-term care regulations and dementia or Alzheimer's care expertise.
- Knowledge of medical terminology, ICD coding (ICD-9/10), medication administration protocols, and infection control practices.
- Strong assessment skills with the ability to perform physical examinations and vital sign monitoring accurately.
- Excellent communication skills to educate patients effectively while maintaining clinical confidentiality policies.
