PreferNP partners with eldercare and skilled nursing facilities throughout the Shenandoah Valley to bring psychiatric and medical care directly to residents through onsite telehealth visits. We are hiring a dependable, take-charge Patient Care Liaison to serve as the in-person presence who makes every visit run smoothly and the trusted connection between our patients, partner facilities, and healthcare providers.
This is a hands-on patient care role, not a tech support or reception position. You'll work directly with elderly residents, many of whom are managing mental health conditions, and you'll typically be the only PreferNP team member onsite. This position is ideal for someone who works confidently and independently, takes initiative, and can solve problems as they arise. If you're someone who naturally steps into a busy environment, organizes the day, and keeps things moving without constant supervision, you'll thrive here.
What You'll Do
- Welcome elderly residents and build trusting relationships that help them feel comfortable during their appointments
- Escort patients to their scheduled visits and remain present during psychiatric telehealth appointments as a supportive, familiar face
- Set up and manage the telehealth equipment (iPad) for every visit and independently troubleshoot basic technical issues
- Coordinate with facility nursing staff and administrators while communicating important patient updates to our providers
- Schedule, track, and manage patient appointments according to each resident's care plan, including following up on missed visits
- Take ownership of the day's schedule by resolving issues, maintaining an efficient workflow, and ensuring appointments stay on time
What We're Looking For

- Clinical or patient-care experience such as:
- Medical Assistant (MA, RMA, CMA, CCMA)
- Certified Nursing Assistant (CNA)
- Licensed Practical Nurse (LPN)
- Patient Access Representative
- Medical Front Desk
- Other hands-on healthcare experience in a clinical setting is strongly preferred.
- Calm, organized, and able to adapt when schedules change or unexpected situations arise.
- Compassion, patience, and professionalism when working with elderly residents and individuals experiencing mental health conditions.
- Comfortable using basic technology, including tablets and video conferencing.
- Reliable transportation and willingness to travel between nearby partner facilities as needed.
- Local to Waynesboro, VA, or the surrounding Shenandoah Valley area.
Pay, Raises & Guaranteed Hours
- Starting pay: $17–18/hour, depending on experience
- Opportunity for a raise after 90 days of dependable attendance and strong performance
Training & Support
We'll provide training on our electronic health record (EHR) system and walk you through our workflow. From there, this is a learn-by-doing role. You'll always have phone support available when needed, but you'll be expected to quickly become comfortable managing the day independently.
