- Registered Nurse (RN)
- Physical Therapist (PT)
Position Overview
As a member of the Luxe Home Health team, you’ll provide one-on-one care to patients in their homes, helping them maintain independence and achieve their health goals. Responsibilities will vary depending on your discipline:
RN Responsibilities:
- Conduct skilled nursing assessments and interventions
- Administer medications and treatments as prescribed
- Provide wound care and patient/family education
- Complete accurate clinical documentation
PT Responsibilities:
- Evaluate and develop individualized treatment plans (PT)
- Implement therapeutic exercises and mobility training
- Monitor and document patient progress
Qualifications
- Active Illinois license in your respective discipline
- CPR Certification
- Valid driver’s license and reliable transportation
- Excellent communication and documentation skills
- Prior home health experience
