As a Hospice Registered Nurse (RN), you deliver compassionate and expert Hospice Care to patients and their families during a pivotal time. - Provides comprehensive Hospice Care, focusing on comfort, dignity, and quality of life for patients.

Conducts thorough Patient Assessment to identify physical, emotional, and spiritual needs.

Develops and implements individualized care plans that address specific patient goals and preferences.

Manages and alleviates symptoms through effective Pain Management and Symptom Management strategies.

Administers medications accurately and safely, adhering to all protocols for Medication Administration.

Offers compassionate Bereavement Support and practical guidance to families before and after a patient's passing.

Provides essential Family Support, educating them on patient care, comfort measures, and the dying process.

Collaborates effectively with physicians, social workers, spiritual counselors, and other healthcare professionals as part of an Interdisciplinary Team Collaboration.

Educates patients and families on disease progression, treatment options, and available resources.

Maintains accurate and timely patient records, documenting all assessments, interventions, and outcomes.
- Holds a current and unrestricted Registered Nurse (RN) license in the state of practice.

Possesses an Associate's Degree in Nursing (ADN) or a Bachelor of Science in Nursing (BSN) from an accredited program.
- Demonstrates at least one year of experience in acute care, home health, hospice, or palliative care settings.

Applies experience in providing direct patient care, including End-of-Life Care.
- Possesses in-depth knowledge of Hospice Care principles and practices.

Applies expertise in Pain Management and Symptom Management techniques.

Proficiently performs Medication Administration and understands pharmacological effects.

Understands and provides effective Bereavement Support.

Communicates clearly and compassionately, both verbally and in writing.

Demonstrates strong organizational skills and the ability to manage a caseload independently.
- Holds a Certified Hospice and Palliative Nurse (CHPN) certification.

Possesses two or more years of experience specifically in hospice nursing.
- Ability to stand, walk, bend, lift, and transfer patients as needed.

Works in various home environments, which may include exposure to different environmental conditions.

Possesses reliable transportation and maintains a valid driver's license for travel to patient homes.

Manages emotional demands associated with providing End-of-Life Care.
