Job Overview
Reliable Quality Care LLC is seeking a dedicated and experienced 245D Care Coordinator (Designated Coordinator) to oversee and coordinate services for individuals receiving Home and Community-Based Services (HCBS). This role ensures all services are delivered in compliance with Minnesota Statutes Chapter 245D, while promoting person-centered outcomes, safety, and quality care.
Key ResponsibilitiesService Coordination & Compliance
- Coordinate and oversee services in accordance with 245D policies and procedures.
- Ensure services align with each person’s Support Plan and Support Plan Addendum.
- Maintain compliance with DHS licensing requirements, documentation standards, and audit readiness.
Person-Centered Planning
- Develop, implement, and review person-centered service plans based on individual needs, preferences, and goals.
- Participate in team meetings with case managers, families, and support teams
- Ensure outcomes support independence, community integration, and quality of life
Staff Supervision & Training
- Provide supervision, coaching, and support to direct support staff.
- Ensure staff complete required 245D training, competency checks, and documentation.
- Promote effective communication and person-centered practices among staff.
Health & Safety Oversight
- Coordinate health services and ensure proper implementation of care plans.
- Monitor medication setup/administration and ensure procedures are followed.
- Ensure incidents, emergencies, and maltreatment reports are handled according to 245D standards.
Documentation & Reporting
- Maintain accurate and timely documentation including:
- Progress notes
- Incident reports
- Service delivery records
- Track and report progress toward individual goals and outcomes.
Admissions & Service Initiation
- Assist with intake, assessments, and service initiation processes.
- Ensure required documents (rights, policies, authorizations) are completed during admission.
Qualifications
- Bachelor’s degree in Human Services or related field(preferred).
- Minimum 1–2 years of experience in HCBS, 245D services, or case coordination.
- Knowledge of Minnesota DHS 245D regulations.
- Experience with person-centered planning and documentation.
- Strong leadership, communication, and organizational skills.
- Valid driver’s license and reliable transportation.
