- Anoxic Brain Injury
- Non-verbal
- Tracheostomy (Trach)
- Percutaneous Endoscopic Gastrostomy (PEG) Tube
Requirements
- Valid FL Driver’s License
- Signed Social Security Care
- Clear/Active LPN/RN License
- Level II Background Check w/ Eligible Status (OIG/NSOPW/Brevard County)
- Valid CPR Card (Online courses not accepted)
- Statement of Health (signed by MD/DO/RN/ARNP/PA within last 6 months)
- Alzheimer’s Certification (CEU)
- Domestic Violence Training (2 hrs. | CEU)
- HIV/AIDS Training (CEU)
- Bloodborne Pathogens/OSHA Training (CEU)
- Florida’s Laws & Rules for Nursing (CEU)
- Prevention of Medical Errors Training (CEU)
- Professional Liability Insurance ($1,000,000 - $6,000,000 coverage limits)
- Additional Documents:
