American Traveler is seeking a Physical Therapist for a home health position providing in-home skilled therapy services to patients in Grand Junction.

Home health setting providing in-home physical therapy services
-
Monday through Friday schedule, 8 am to 5 pm
-
Guaranteed 40 hours per week
-
Serves patients in the Grand Junction community
-
One-on-one, patient-centered visits
-
Uses Homecare Homebase (HCHB) for documentation
-
Current, unrestricted Colorado PT license required (license required if applicable)
-
Graduation from an accredited Physical Therapy program
-
Valid driver's license and reliable transportation required
-
Ability to practice independently in a home-based setting
-
Proficiency or willingness to learn HCHB
-
No minimum years of experience required
-
Home health experience preferred
-
Familiarity with Medicare documentation and home health regulations preferred
-
Experience with geriatric and medically complex populations preferred
-
Perform physical therapy visits including evaluations, Start of Care, reassessments, recertifications, and discharge visits
-
Develop and implement individualized plans of care based on patient needs and physician orders
-
Provide skilled interventions such as gait training, balance training, therapeutic exercise, mobility training, and fall prevention
-
Educate patients and caregivers on treatment plans, home exercise programs, and safety strategies
-
Communicate patient progress and condition changes to the interdisciplinary care team
-
May provide clinical oversight of Physical Therapist Assistants as applicable and permitted under Colorado law and agency policy

Collaborate with RNs, PTAs, OTs, STs, and agency leadership on care coordination
-
Ensure documentation complies with Medicare Conditions of Participation and HIPAA requirements

Mission-driven home health services with supportive clinical and administrative coordination
