Job Summary
We are seeking a dedicated Licensed Practical Nurse (LPN) to support individuals with intellectual and developmental disabilities in our ICF residential program. Under the supervision of the Director of Nursing, this position provides direct nursing care, medication administration, health monitoring, and coordination of medical services. The LPN ensures compliance with all federals, state, and agency regulations while maintaining accurate medical records and supporting a safe, person‑centered environment.
Responsibilities
- Provide direct nursing care and follow all facility policies and procedures
- Administer medications, treatments, and first aid as ordered
- Monitor consumer health status and document all changes
- Schedule and accompany consumers to medical and dental appointments
- Maintain medication supplies, check expiration dates, and keep storage areas organized
- Complete required medical forms, records, and weekly/monthly documentation
- Assist with quarterly pharmacy inspections and annual plan meetings
- Provide health‑related training to staff and consumers
- Take and record vital signs and weights as required
- Maintain a clean and orderly nursing area
- Communicate promptly about medical concerns or discrepancies to the Director of Nursing
- Collaborate with other disciplines to support consumer services
- Follow all federal, state, and agency nursing regulations
- Perform other duties as assigned by the Director of Nursing
Requirements
- Valid state licensure as a Licensed Practical Nurse (LPN) or equivalent credential
- Proven experience working in diverse healthcare environments such as medical-surgical units, long-term care facilities, hospice care, or emergency rooms
- Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, or Athenahealth for accurate medical documentation
- Knowledge of medical terminology, anatomy, physiology, and ICD coding (ICD-9/10) for accurate record keeping and billing purposes
- Experience with IV insertion, catheter care, medication administration—including injections—and basic life support (BLS) certification
- Familiarity with infection control practices and aseptic techniques to ensure patient safety
- Strong communication skills to effectively interact with patients of all ages—including geriatric patients—and coordinate with caregivers and healthcare teams
- Ability to work in fast-paced environments such as ICU settings or Level I Trauma Centers; experience in acute or emergency medicine is highly valued
- Knowledge of long-term care regulations and experience working with individuals with disabilities is preferred
PERFORMANCE REQUIREMENTS:
Knowledge, Skills and Abilities: Ability to work independently and make decisions; must possess excellent writing and communication skills, ability to deal tactfully with others and treat the consumers with dignity and respect.
Education: Completion of an accredited school of Practical Nursing; current LPN license in good standing.
Experience: minimum of one (1) year experience with people with intellectual/developmental disabilities
Certificate/License: South Carolina LPN license from the SC Board of Nursing - active
Other: Must hold a valid driver’s license and have a satisfactory driving record and a satisfactory SLED report.

Pay: $27.00 - $30.00 per hour
