JOB DETAILS
*Full-Time: (4-5 days per week), OR *Part- Time: 3-4 patients, Monday - Friday, plus $15 mileage pay between each visit.
Rotating on-call weekend visits once per month, if scheduled by the agency, are paid at 2x pay.
You will be responsible for delivering high-quality nursing care to assigned patients in their homes.
Starting pay based on verifiable home health experience with positive employment history references. Payroll is paid every 14 days.
A 100% quality review of submitted clinical documentation provides an added layer of compliance and security for our patients, doctors, and nurses.

Must provide verifiable job references (home health setting preferred), pass a criminal background check, and random drug testing as required by the Texas Administrative Code for Home Health Agencies.
Responsibilities:
- Executing a Plan of Care for patients in their homes
- Applicants must be able to complete nursing clinical notes, communication notes, and any physician orders at the time the patient visit is completed.
- Provide patient care and treatment according to M.D. orders.
- Update the patient's Primary Care Physician/Specialty Physicians as necessary with regard to the patient's condition at the time of assessment.
- Generate accurate M.D. orders upon receipt when collaborating with physicians and their staff.
- Collaborate with Registered Nurses and office staff on the patient's health status
- Educate and update patients/caregivers and patient's doctors on medical treatments, medications, and upcoming appointments.
- Prepares equipment, disinfects service products for patient treatments or assessments using aseptic techniques as ordered by physicians.
- Obtain signatures from patients/caregivers on required forms.
- Must attend weekly telephonic/virtual Case Conference every Wednesday, variable time duration, scheduled between 3 pm - 4 pm. This time period also incorporates education/in-services on company policies, procedures, best practices, and changes in home health regulations.
Qualifications:
- Previous experience in Home Health nursing is required.
- WellSky/Kinnser electronic health records experience is a plus.
- Strong time management skills are required to complete nursing encounters and requisite documentation.
- Capability to proficiently use company-issued Apple iPad for submission of clinical documentation.
- Familiarity with Kinnser/WellSky Software is preferred.
- Ability to build rapport with patients, nurses, office staff, patients, and their caregivers.
- Compassionate and caring demeanor.
- Excellent written and verbal communication skills.
- Must have current: driver's license, auto insurance, CPR license, Texas B.O.N. license.
