LICENSED PRACTICAL NURSE (LPN)
Position Summary
On-campus nursing presence and Medical department in-house reporting lead; medication administration under RN/MD supervision. Prescribing sits with the off-site NP and Medical Director.
Required Qualifications:
- Active Indiana Licensed Practical Nurse (LPN) license
- CPR/BLS certification
- Experience in behavioral health or residential treatment preferred
- Knowledge of medication administration, controlled-substance control, and infection control
Pre-Employment Requirements:
- Drug test at Hancock Gateway Lab
- Criminal background check must clear before first day
- OIG LEIE screening
- TB skin test or proof within past year
- Immunizations
Job Responsibilities (derived from ARISE policies):
- Serve as the on-campus nursing presence and the Medical department's in-house reporting lead
- Administer medications per orders and the MAR under Medical Director / Nurse Practitioner orders; the off-site RN provides nursing oversight and the Medical Director holds ultimate medical authority
- Support admission nursing assessments and vital signs
- Maintain controlled-substance counts, reconciliation, and secure storage
- Monitor withdrawal (CIWA/COWS support) and escalate per protocol
- Conduct ongoing health monitoring and daily symptom screening
- Document and report medical incidents and medication errors as critical incidents
- Support infection control and TB screening
- Maintain accurate nursing documentation and shift handoff
- Escalate prescribing and medical decisions to the off-site NP and Medical Director
- Maintain Indiana LPN licensure and continuing education

Pay: $27.00 - $35.00 per hour
