Role Overview:We are seeking a compassionate and experienced Registered Nurse to join our team. This remote role focuses on providing comprehensive healthcare services to patients with uncontrolled blood sugars, including medication management, health education, and coordination of care. The ideal candidate will have a strong background in primary care and a passion for improving health outcomes in underserved communities.
Key Responsibilities:
- Patient Assessment and Care:
- Conduct thorough physical assessments and health histories of patients with uncontrolled blood sugars.
- Develop and implement personalized care plans, including medication management and lifestyle recommendations.
- Monitor patient progress and adjust care plans as needed.
- Health Education and Counseling:
- Provide patient education on diabetes management, including medication adherence, nutrition, and lifestyle modifications.
- Offer counseling and support to help patients navigate challenges related to their health and well-being.
- Empower patients to take control of their health and make informed decisions about their care.
- Collaboration and Coordination:
- Collaborate with other healthcare providers, including physicians, dietitians, and mental health professionals, to ensure coordinated care.
- Communicate effectively with patients, caregivers, and other members of the healthcare team.
- Coordinate referrals to specialists and community resources as needed.
- Data Management and Documentation:
- Maintain accurate and up-to-date patient records in the EHR or case management system.
- Document patient assessments, care plans, and interventions in a timely and thorough manner.
- Ensure compliance with all regulatory and documentation requirements.
- Quality Improvement and Program Development:
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
- Contribute to the development and evaluation of programs and services aimed at improving medication adherence and health literacy.
- Stay informed about current trends and best practices in diabetes management and primary care.
- Professional Development and Compliance:
- Maintain licensure and certification as a Nurse Practitioner in applicable states.
- Participate in ongoing education and training to enhance clinical skills and knowledge.
- Adhere to all company policies, procedures, and ethical standards.
Qualifications:
- Active Registered Nurse (RN) license in the state of Florida (multi-state licensure is a plus)
- Associate or Bachelor’s degree in Nursing (BSN preferred)
- Minimum of 2 years of clinical nursing experience, preferably in primary care, community health, or chronic condition management
- Strong understanding of chronic conditions like diabetes and hypertension
- Excellent communication, patient education, and care coordination skills
- Comfortable using EHR systems and remote monitoring toolsPassionate about improving health outcomes for underserved and high-need populations
