Job Summary
Join our dedicated hospice care team as a Weekend On-Call Licensed Practical Nurse (LPN), where your expertise will provide compassionate, high-quality nursing care to patients in their homes during weekend hours. This vital role ensures seamless patient support outside regular hours, offering comfort and relief to patients and their families while maintaining adherence to all regulatory standards. As an on-call LPN, you will be the primary point of contact for urgent patient needs, utilizing your clinical skills and critical thinking to deliver exceptional end-of-life care with empathy and professionalism.
Duties
- Respond promptly to on-call patient needs during weekend shifts, providing assessments and interventions as required.
- Conduct comprehensive patient evaluations, including vital signs, symptom management, and medication administration, ensuring comfort and safety.
- Collaborate with interdisciplinary teams to coordinate care plans, discharge planning, and follow-up services tailored to each patient's unique needs.
- Manage complex symptom control such as pain, nausea, dyspnea, and other distressing symptoms associated with hospice patients.
- Document all patient interactions accurately within electronic health record (EHR) systems like Epic or Athenahealth, ensuring compliance with HIPAA and confidentiality policies.
- Provide education and emotional support to patients and families regarding disease progression, medication use (including IV infusions), and advanced directives.
- Maintain clear communication with physicians, case managers, and other healthcare providers to ensure continuity of care across settings such as skilled nursing facilities or assisted living communities.
Experience
- Valid LPN license with current state licensure required.
- Prior experience in hospice or palliative care preferred; experience working with individuals with disabilities or dementia is a plus.
- Demonstrated proficiency in medical documentation using EMR/EHR systems such as Cerner or eClinicalWorks.
- Strong clinical skills including IV infusion management, catheter care, medication administration, and basic life support (BLS).
- Experience working in diverse settings such as hospital units (ICU, emergency room), long-term care facilities, or home health environments.
- Knowledge of critical care principles including acute pain management, airway management, infection control protocols, and trauma response (Level I & II trauma center experience advantageous).
- Familiarity with ICD coding (ICD-9/10), CPT coding for procedures, and diagnostic evaluation techniques.
- Excellent caregiver communication skills with the ability to assess patient physiology accurately and respond effectively in urgent situations like triage or emergent changes in condition.

Pay: $27.00 - $29.00 per hour
