Small, 11 bed congregate living health facility that operates with the clinical standards of a subacute/SNF. We are seeking an experienced RN Charge Nurse who values hands-on care, strong teamwork, and a manageable nurse-to-patient ratio.
As the RN Charge Nurse, you will play a key leadership role on shift while still staying connected to resident care. This is an ideal position for an RN who enjoys autonomy, collaboration, and providing high-quality, patient-centered care in a close-knit setting.

Schedule Requirements
- 12-hour shifts (AM and/or PM)
- Per Diem or Part-time availability
- Some weekend availability for part-time staff
Responsibilities
- Serve as Charge Nurse during assigned shifts
- Provide direct nursing care based on individualized care plans
- Perform resident assessments and monitor changes in condition
- Supervise and support LVNs and CNAs on shift
- Administer medications and treatments safely and accurately
- Coordinate and complete admissions and discharges
- Communicate with physicians and interdisciplinary team members
- Ensure timely, accurate documentation in PointClickCare
- Respond promptly to clinical issues and emergencies
Requirements
- Active California RN license in good standing
- Minimum 1 year of SNF or Subacute experience
- Curent BLS Certification
- Experience with PointClickCare
- Strong leadership, clinical judgement, and decision making skills
- Ability to work independently and manage shift responsibilities
- Trach and Vent experience

Pay: $50.00 - $53.00 per hour
