We are currently seeking a licensed Physical Therapist (PT) to join our dedicated and collaborative therapy team. This is a full-time opportunity to provide high-quality therapy services in a supportive, resident-centered environment.
The Physical Therapist works under the direction of the Director of Rehabilitation and is responsible for evaluating, planning, and implementing individualized physical therapy programs for residents. This role focuses on improving mobility, function, and overall quality of life through evidence-based therapeutic care.
Provide physical therapy services to residents throughout the facility
Conduct comprehensive physical therapy evaluations based on physician orders
Develop and implement individualized treatment plans, including short- and long-term goals
Deliver skilled therapy interventions and modify treatment plans as clinically indicated
Participate in resident assessments and interdisciplinary care planning
Collaborate with nursing, therapy, and medical teams to support optimal resident outcomes
Graduate of an accredited Physical Therapy program (Bachelor’s, Master’s, or Doctorate) recognized by APTA
Current Physical Therapist license in good standing (required)
Experience working with the geriatric population preferred
Strong clinical, communication, and teamwork skills
