The Atrium at Navesink Harbor is seeking a Director of Social Services/Admissions to join our team!
The Atrium at Navesink Harbor is a Senior Living Community located in Red Bank, NJ. At Springpoint, the Director of Social Services/Admissions assess individual psychosocial and emotional needs of residents through interview process within fourteen days of admission and develop individual plan of care with interdisciplinary care plan team members with a focus on desires of resident.
Responsibilities:
- Meet with new residents to discuss availability of Advance Directive, answer questions, provide Health Care Center Resident Handbook and Resident Bill of Rights. Obtain signatures for Bill of Rights, Ombudsman Disclosure Form, Chaplain Authorization Form, Laundry Form upon admission, and to send Bill of Rights to family members.
- Secure copy of Advance Directive, detailing type of directive, date signed, etc. Provide copy of case note to Administrator's secretary. Assist resident in signing Advance Directive if desired.
- In-service staff in Health Care Center, Assisted Living, and Memory Unit (including housekeeping and activities) on abuse prevention, detection and reporting annually.
- Develop and facilitate ongoing staff education in areas related to social service including such original programs as "Psychosocial Ethics".
Education and Experience:
- College Degree and MSW Required
- Minimum of 5 years of experience
- Must be licensed by the New Jersey Social Work Licensing Board

Pay Range: $80,000-$90,000
