Case Manager

Job Summary
Englewood Heroes is seeking a compassionate and organized Case Manager to support individuals and families impacted by violence and other life challenges. The Case Manager will assess participant needs, develop individualized service plans, coordinate referrals, and provide ongoing support to help participants achieve stability and self-sufficiency.
Key Responsibilities
- Conduct participant assessments and develop individualized case plans.
- Connect participants to community resources, including housing, employment, education, healthcare, mental health, and financial assistance.
- Monitor participant progress and provide ongoing advocacy and follow-up.
- Maintain accurate and timely case notes, documentation, and program records.
- Collaborate with outreach staff and community partners to coordinate services.
- Assist participants in setting and achieving personal goals.
- Ensure compliance with program policies, confidentiality requirements, and reporting standards.
Qualifications
- Experience in case management, community outreach, or human services preferred.
- Strong communication, organizational, and problem-solving skills.
- Ability to build trust with diverse populations.
- Proficiency in Microsoft Office and electronic documentation.
- Knowledge of community resources and trauma-informed care is a plus.
