Overview
Join our dedicated healthcare team as a Licensed Practical Nurse (LPN), working Monday through Friday from 1:00 pm to 7:00 pm in a vibrant and supportive environment. Responsibilities
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Epic or Cerner.
- Administer medications and injections accurately following prescribed protocols, including IV infusions and medication management for pain control or chronic conditions.
- Assist with diagnostic evaluations, specimen collection, and processing while maintaining strict adherence to infection control policies.
- Provide wound care, catheterization, tube feeding, and ventilator management as needed for patients with complex medical conditions.
- Collaborate with interdisciplinary teams to develop and implement personalized care plans aligned with long-term care regulations and best practices in geriatrics and dementia care.
- Educate patients and caregivers on health management strategies, including Alzheimer's care, memory support, and post-acute discharge planning.
- Document all patient interactions thoroughly in EMR/EHR systems such as Athenahealth or eClinicalWorks, ensuring compliance with HIPAA and clinical confidentiality policies.
Qualifications
- Valid Licensed Practical Nurse (LPN) license with current state certification.
- Proven experience working with geriatric patients in settings such as skilled nursing facilities, assisted living communities, or long-term care environments.
- Strong knowledge of medical terminology, anatomy, physiology, and ICD coding (ICD-9/10).
- Experience administering injections, performing venipuncture or phlebotomy, and managing sterile procedures like suturing or catheter care.
- Familiarity with EMR/EHR systems such as Epic or Cerner; prior experience with hospice care or home & community-based services is a plus.
- Ability to work independently and as part of a multidisciplinary team in fast-paced environments like hospital medicine or urgent care settings.
- Excellent communication skills for effective caregiver communication and patient education while maintaining clinical confidentiality policies.
