Responsibilities
- Conduct thorough patient assessments to evaluate individual needs, including medical history, lifestyle, and dietary habits.
- Develop and implement tailored care plans focusing on diabetes management, nutrition support, weight loss, and chronic disease control.
- Provide expert nutrition counseling and medical nutrition therapy (MNT) to optimize health outcomes for patients with specific dietary needs.
- Utilize motivational interviewing techniques to motivate behavioral change and foster patient engagement in self-care routines.
- Educate patients on blood glucose monitoring, insulin administration, tube feeding procedures, and other diabetes-related therapies.
- Collaborate effectively with healthcare professionals such as physicians, dietitians, nurses, and pharmacists to coordinate comprehensive care strategies.
- Document all patient interactions accurately within electronic health records (EHR/EHR systems) while maintaining confidentiality and compliance with healthcare regulations.
- Supervise or mentor junior staff or interns in clinical nutrition practices and patient education methodologies.
- Stay current with advancements in physiology knowledge, diagnostic evaluation methods, and clinical nutrition best practices related to diabetes care.
- Manage long-term care plans for geriatric populations or individuals in acute or hospital settings requiring specialized nutritional support.
Skills
- Proven experience in health coaching with a focus on chronic disease management and patient empowerment.
- Demonstrated supervising experience within clinical or dietary departments.
- Strong knowledge of physiology, medical nutrition therapy (MNT), and nutritional analysis techniques.
- Expertise in managing tube feeding protocols and providing specialized nutrition support for complex cases.
- Skilled in conducting diagnostic evaluations to inform personalized care plans.
- Proficient in motivational interviewing techniques to facilitate behavioral change effectively.
- Excellent client communication skills with the ability to educate diverse populations about nutrition counseling and disease management strategies.
- Familiarity with EMR/EHR systems for documentation, care coordination, and medical record management.
- Experience working with individuals with specific dietary needs such as geriatrics or those requiring weight loss interventions.
- Background in hospital or long-term care environments with acute care experience preferred.
- Knowledge of dietary department operations within healthcare settings is a plus.
