CaliberHealth Plus is seeking a compassionate. Must hold current CHHA certification, have experience with bed-bound patients, and be able to understand English to communicate with family. Reliable, patient, and compassionate individuals only. Background check and references are required. Certified Home Health Aide (CHHA) to care for a bed-bound patient, six days per week. Responsibilities include bathing, grooming, dressing, feeding, safe transfers/repositioning, toileting care, and monitoring the patient's condition
Responsibilities
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Support medication administration following prescribed protocols while maintaining strict patient confidentiality in compliance with HIPAA regulations
- Observe and report changes in patient condition through detailed documentation and communication with healthcare teams
- Provide specialized care such as catheter management, dementia and Alzheimer’s care, end-of-life support, and memory care services
- Assist with patient transfer methods using equipment like Hoyer lifts or transfer belts to ensure safety during mobility tasks
- Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe environment for clients
Qualifications
- Valid certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA) with current licensure where applicable
- Proven experience working with geriatric patients, individuals with disabilities, or those requiring hospice or dementia care
- Familiarity with medical terminology, medical documentation practices, and home health care regulations
- Ability to perform patient personal care assistance safely and effectively while maintaining professionalism and empathy
- Knowledge of patient transfer methods and the use of assistive devices such as Hoyer lifts or gait belts
- Experience working with EMR/EHR systems for accurate record keeping and documentation
- Strong communication skills to coordinate effectively with clients, families, and healthcare providers
- Physical ability to lift heavy objects when necessary and perform caregiving tasks that require stamina
