Pay Range: $75– $110 per visit (based on experience and case type)

Position Overview
We are seeking a motivated Physical Therapist to provide home-based care to patients with a variety of functional impairments. This role involves evaluating patients, developing individualized treatment plans, and delivering high-quality care aimed at maximizing independence and mobility. You’ll also collaborate with the broader care team and maintain accurate clinical documentation.
Key Responsibilities
- Perform initial evaluations and ongoing assessments to determine patient needs
- Develop and implement personalized treatment plans
- Deliver skilled therapy services focused on strength, mobility, and functional outcomes
- Complete documentation in a timely and compliant manner
- Communicate effectively with patients, families, and interdisciplinary team members
- Supervise PTAs and therapy aides when applicable
- Participate in care coordination and case discussions as needed
Qualifications
- Graduate of an accredited Physical Therapy program
- Active Michigan Physical Therapist license
- Home health experience preferred
Skills & Attributes
- Strong clinical decision-making and problem-solving ability
- Self-motivated and comfortable working independently
- Excellent communication and interpersonal skills
- Familiarity with EMR systems for efficient documentation and communication within healthcare teams. Kinnser/Wellsky experience highly preferred.
- Ability to safely assist patients with mobility and transfers (with appropriate equipment)
