Your Role:
- Conduct in-home visits, assessing and developing treatment plans tailored to patient needs
- Educate patients and families on exercises and home safety
- Collaborate with families and healthcare professionals to ensure continuity of care
- Maintain detailed and timely documentation of patient progress and care plans

Requirements:
- Active Florida Physical Therapy license
- Experience with seniors preferred but not required
- Valid driver’s license and reliable transportation
- Professional Liability Insurance
