Atrio Home Care— a partnership between Holland Home, Brio and Rest Haven— is seeking a compassionate Licensed Practical Nurse (LPN) to provide in-home care throughout the Lakeshore region. This role focuses on delivering high-quality, one-on-one care to clients across Ottawa and Allegan Counties.
Key Responsibilities

Deliver skilled nursing care under RN supervision, including wound care, dressing changes, catheter care, and medication/injection administration

Follow and support individualized Plans of Care in collaboration with physicians and the care team

Provide specialized nursing services such as preventive care (skin care, pressure relief) and chronic disease management

Educate clients and caregivers on disease processes, self-care, and health promotion to encourage independence

Maintain accurate, timely documentation using the Matrix EMR system (iPads provided)
Schedule & Location
Territory: Ottawa & Allegan Counties
Schedule: Full-time or part-time opportunities available
Hours: Day shift (8:00 AM – 5:00 PM)
Weekend/On-Call/Ho;idays: None
Qualifications

Current Michigan LPN license
-
Valid driver’s license, reliable transportation, and proof of auto insurance

1–2 years of acute care or skilled nursing experience (home health preferred)

Passion for delivering exceptional care to older adults
