Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform individualized care plans.
- Administer medications and injections accurately following physician orders while monitoring for adverse reactions or side effects.
- Assist with medical procedures such as IV infusions, catheterizations, wound care, and sterile dressing changes using aseptic techniques.
- Manage patient documentation efficiently within Electronic Health Record (EHR) systems like PCC, ensuring compliance with HIPAA and long-term care regulations.
- Provide specialized care for patients with complex needs such as dementia, Alzheimer's disease, or those requiring memory or behavioral health management.
- Support triage activities by evaluating patient symptoms and determining urgency levels for appropriate treatment pathways in urgent or emergency settings.
- Collaborate with interdisciplinary teams to coordinate discharge planning, case management, and post-acute care transitions for optimal patient outcomes.
Open Shift:
7pm-7am (Our nursing shifts are 12 hours)
Skills
- Proven experience working with diverse populations including geriatric patients, individuals with developmental disabilities, or those in hospice or palliative care settings.
- Ability to perform physical examinations and vital signs assessment with attention to detail and clinical confidentiality policies.

Pay: $28.00 - $33.50 per hour
