The JCOD Care Management (JCM) Case Manager provides comprehensive, field-based case management services to justice-involved individuals, with a focus on successful community reentry, stabilization, and long-term self-sufficiency. This role includes care coordination, system navigation, outreach, advocacy, and participant engagement using a trauma-informed and participant-centered approach.
The Case Manager delivers services in alignment with JCOD Care Management (JCM) and Enhanced Care Management (ECM) program requirements, including adherence to Medi-Cal, documentation, billing, and care coordination standards. The position is responsible for ensuring timely, accurate, and compliant service delivery in accordance with County, State, and contractual obligations.
The Case Manager’s primary responsibility is direct participant care and caseload management, including engagement, service delivery, and documentation. All other duties are secondary to maintaining an active caseload and meeting program service requirements.
Essential Duties & Responsibilities
Caseload & Productivity Expectations
- Maintain an active caseload (28-30 participants a month) in accordance with program requirements and funding expectations.
- Prioritize direct participant care, engagement, and service delivery as the primary responsibility of the role.
- Meet minimum contact, service delivery, documentation, and billing expectations required by the JCM/ECM program.
- Complete all required participant interactions and documentation within established timeframes.
- Ensure productivity aligns with program performance metrics, funding requirements, and organizational expectations.
Participant Engagement & Support
- Build trusting, professional relationships with participants to support successful reentry and long-term stability.
- Mentor, motivate, and empower participants through goal setting, coaching, and ongoing support.
- Conduct outreach and engagement in the field, including homes, streets, hospitals, correctional facilities, and community locations.
- Provide ongoing follow-up to ensure continued participation and engagement in services.
Assessment & Care Planning
- Conduct comprehensive assessments to identify participant strengths, needs, and barriers.
- Develop individualized care plans addressing housing, healthcare, behavioral health, employment, and social support needs.
- Support participants in establishing and achieving short- and long-term goals related to self-sufficiency.
Care Coordination (ECM-Aligned)
- Coordinate care across physical health, behavioral health, substance use treatment, and social service systems.
- Collaborate with Managed Care Plans (MCPs), healthcare providers, and community partners to ensure continuity of care.
- Participate in multidisciplinary case conferencing and care team meetings.
- Provide warm hand-offs and supported referrals to ensure successful linkage to services.
System Navigation & Advocacy
- Assist participants in navigating healthcare, behavioral health, and social service systems.
- Provide appointment accompaniment, transportation coordination, and service linkage support.
- Advocate on behalf of participants to access services, remove barriers, and achieve care plan goals.
Health Education & Support
- Provide education on chronic disease management, medication adherence, mental health, substance use, and self-care.
- Support participants in understanding and utilizing available healthcare and social service resources.
Documentation, Billing & Compliance
- Complete timely, accurate, and compliant documentation of all participant interactions, services, and encounters.
- Enter all services into required case management and billing systems to support reimbursement and invoicing.
- Ensure documentation meets Medi-Cal, ECM, County, and audit standards, including medical necessity and service eligibility.
- Maintain organized and up-to-date participant records in accordance with program requirements.
Data Tracking & Reporting
- Track participant progress, service delivery, and outcomes to support program evaluation and reporting.
- Assist in data collection and reporting required by funders, including County and State agencies.
- Support quality assurance activities and program monitoring efforts.
Crisis Intervention & Safety
- Respond to participant crises using de-escalation and trauma-informed approaches.
- Provide support for mental health, substance use, housing instability, and other urgent needs.
- Follow all agency safety protocols during field-based work and outreach activities.
Community Collaboration
- Build and maintain relationships with community organizations, providers, and stakeholders.
- Coordinate with partner agencies to expand access to services and resources.
Compliance & Confidentiality
- Maintain compliance with all applicable Federal, State, and local regulations, including HIPAA, Medi-Cal, and County contractual requirements.
- Protect participant confidentiality and ensure secure handling of protected health information (PHI).
- Adhere to all organizational policies, procedures, and ethical standards.
Additional Duties
- Perform other related duties as assigned, provided such duties are reasonably within the scope of the position and consistent with program requirements and funding guidelines.
Minimum Qualifications
- Bachelor’s degree in social work, public health, psychology, or a related field, or equivalent experience.
- Experience working with justice-involved, unhoused, or high-acuity populations preferred.
- Knowledge of healthcare systems, community resources, and social service navigation.
- Experience with case management, care coordination, or outreach services.
- Strong documentation, organizational, and time management skills.
- Ability to work independently and in field-based environments.
- Valid California driver’s license and reliable transportation (if required).

Work Environment
The employee will work partially in an office environment where noise levels are typically quiet, generally at or below 85 decibels, and will also work in the field where exposure to higher noise levels may occur. Field work may involve exposure to heat, variable weather conditions, road hazards, vibration, and hazardous physical substances or fumes, in accordance with applicable safety regulations and protocols. While conducting field work and/or outreach activities, employees may be required to work in public spaces and areas where unhoused individuals are present. These environments may involve unsanitary conditions, unpredictable circumstances, and interactions with members of the public that may require situational awareness, professionalism, and adherence to safety procedures. The employee must be able to perform the essential functions of the position, with or without reasonable accommodation, in compliance with applicable laws and safety standards.
Physical Requirements
Work is performed in both office and field environments. Field work may include exposure to varying weather conditions, public settings, and environmental elements, as well as potential occupational hazards such as exposure to blood or other bodily fluids, consistent with applicable health and safety regulations and established safety protocols. The position requires the ability to perform a range of physical activities, which may include sitting, standing, walking, bending, reaching, twisting, kneeling, and grasping; using a computer, keyboard, and related equipment; and lifting and carrying materials weighing up to 30 pounds. Field-based duties may involve walking on uneven or slippery surfaces, navigating stairs or inclines, and exposure to dust, construction materials, or other environmental conditions typical of community-based work.
The position may require driving a personal or agency vehicle for work-related purposes, in accordance with organizational policies. Pay: $20.00 - $30.00 per hour
