We are seeking a compassionate, organized, and dependable Social Services Assistant to join our skilled nursing facility team in Florida. The ideal candidate will have experience working in a long-term care/skilled nursing environment and hands-on experience with PointClickCare (PCC).
Key Responsibilities
- Assist the Social Services Director with the day-to-day functions of the Social Services Department.
- Provide support to residents and families regarding psychosocial, emotional, and adjustment needs.
- Assist with new resident admissions, assessments, care planning, and discharge planning.
- Maintain accurate and timely documentation in PointClickCare (PCC).
- Participate in care plan meetings and communicate resident needs and concerns to the interdisciplinary team.
- Assist residents and responsible parties with facility-related questions, concerns, and requests.
- Help coordinate communication between residents, families, staff, physicians, and outside agencies.
- Assist with resident rights, grievances, and concerns in accordance with facility policies and applicable regulations.
- Maintain accurate resident records and documentation.
- Assist with arranging services and resources to support residents' needs.
- Promote resident dignity, independence, privacy, and quality of life.
- Maintain confidentiality and comply with all applicable federal and Florida long-term care regulations.
Qualifications
- Previous experience in a long-term care/skilled nursing facility required.
- PointClickCare (PCC) experience required.
- Previous experience in social services, healthcare, senior living, or a related setting preferred.
- Strong communication, interpersonal, and organizational skills.
- Compassionate and professional approach when working with residents and families.
- Ability to work effectively as part of an interdisciplinary team.
- Strong attention to detail and accurate documentation skills.
- Ability to maintain confidentiality and handle sensitive resident information.
- Knowledge of long-term care regulations and resident rights is a plus.
