Job Overview
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. In this role, you will be responsible for coordinating patient care, ensuring effective communication among healthcare providers, and facilitating the discharge planning process. You will work closely with patients and their families to navigate the complexities of their healthcare journey, particularly in pediatrics, geriatrics, and various hospital settings.
Responsibilities
- Assess patient needs and develop individualized care plans based on medical history and current health conditions.
- Coordinate with multidisciplinary teams to ensure comprehensive patient care throughout their hospital stay.
- Manage medical records and documentation accurately to reflect patient progress and treatment plans.
- Facilitate discharge planning by preparing patients for a smooth transition from hospital to home or other care settings.
- Provide education and support to patients and families regarding medical terminology, treatment options, and follow-up care.
- Monitor patient outcomes and adjust care plans as necessary to meet evolving health needs.
- Collaborate with healthcare professionals in various departments including ICU, emergency room, OR, and pediatrics to optimize patient care.
Skills
- Strong knowledge of medical terminology and proficiency in managing medical records.
- Experience in pediatrics, geriatrics, hospital medicine, or critical care settings such as ICU or emergency room.
- Excellent communication skills for effective interaction with patients, families, and healthcare teams.
- Ability to perform discharge planning effectively while considering the unique needs of each patient.
- Strong organizational skills with attention to detail in documenting patient care processes.
