Case Management Specialist
Reports to: Sr. Director of Operations & Strategy
Status: Full-Time
Schedule: 40 hours per week, evenings and weekends as needed.
Salary: $40,000–$45,000 per year, commensurate with relevant experience and CHW certification.
Position Summary:
This position will lead and support LAUP’s Refugee Support Services (RSS), Community Resource Navigation (CRN), Mental Health, Workforce Support, and community-based service initiatives through comprehensive case management, resource coordination, outreach, workshop coordination, and grant-funded program implementation. Services may be provided in-office, onsite at workforce and corporate partner locations, within community settings, or through outreach and partner engagement activities.
The role provides wraparound support to individuals and families navigating barriers related to healthcare, mental health, housing, employment, education, transportation, financial stability, and other social determinants of health. This position plays a critical role in maintaining high-quality and compliant case management systems across multiple grant-funded programs. The role requires strong case management experience and a high level of accuracy and accountability in client intake, eligibility determination, case documentation, case notes, supporting documentation collection, data tracking, file management, reporting, and service coordination in alignment with grant, organizational, and compliance requirements.
The position serves as a liaison between clients, healthcare providers, workforce partners, community organizations, and public agencies to coordinate culturally responsive services, facilitate warm handoffs, and connect individuals to critical community resources. Additionally, this role supports outreach and community engagement activities, workshop coordination, mental health and workforce initiatives, grant reporting and compliance efforts, and data quality processes that strengthen service delivery systems and promote long-term stability and self-sufficiency for the communities LAUP serves.
Core Responsibilities
- Manage comprehensive client intake, case management, and resource navigation services both in-office and onsite at workforce and corporate partner locations, community settings, workshops, outreach events, and partner organizations
- Provide ongoing case management support to newcomers (including Latino/Latinx clients), recurring clients, and walk-ins via phone, email, and in-person interactions while addressing social determinants of health
- Conduct comprehensive intake assessments to identify client needs related to healthcare, mental health, housing, employment, education, transportation, finances, food access, and other stabilization needs
- Determine client eligibility for services and programs while ensuring all required intake forms, releases, supporting documentation, verification materials, and case records are collected, completed, and maintained accurately
- Maintain timely, accurate, and compliant case files, including case notes, service plans, referrals, follow-ups, eligibility documentation, outcome tracking, and supporting records within RSS CDM, Casebook, CRN tracking systems, and other required reporting platforms
- Manage reporting, data tracking, file audits, and compliance activities across Community Resource Navigation (CRN), Refugee Support Services (RSS), Mental Health, Workforce Support, and other grant-funded initiatives
- Assist in developing and improving case management workflows, documentation standards, reporting processes, and program management systems to support high-quality service delivery and grant compliance
- Advocate on behalf of clients, facilitate warm handoffs, and coordinate referrals with healthcare providers, workforce partners, schools, public agencies, mental health providers, and community-based organizations
- Build and maintain collaborative relationships with community organizations, service providers, faith groups, employers, local businesses, and public agencies to strengthen referral pathways and expand community resource networks
- Collaborate with regional coalitions and community collaboratives to identify local needs, service gaps, and support community needs assessments and coordinated service delivery efforts
- Support outreach, workshop coordination, and community engagement activities that increase awareness of available services and community resources
- Maintain confidentiality and handle sensitive client information in accordance with organizational policies, grant requirements, and applicable privacy standards
- Maintain a valid driver’s license and reliable personal vehicle for travel to partner meetings, client appointments, outreach activities, workshops, and community events
Qualification
- Bachelor’s degree in Social Work, Human Services, or related field
- Community Health Worker (CHW) Certification is not required upon hire but must be successfully obtained within six (6) months of employment as a condition of continued employment.
- Bilingual (English/Spanish) with proven ability to work with diverse populations and act as a cultural liaison
- 2–3 years of community outreach and case-management experience
- Strong commitment to the organization’s mission, vision, and principles of diversity, equity, and inclusion
- Excellent problem-solving, analytical, communication, and organizational skills; proactive, adaptable, and able to manage competing priorities
- Proficient with Google Suite, Casebook, social media, and database software, with a quick aptitude for new tools
- Flexibility to work outside regular business hours as needed
