Three Rivers Hospice in Jefferson City is seeking a compassionate and highly skilled Hospice Registered Nurse (RN) Case Manager to join our dedicated team. In this vital role, you will provide comprehensive nursing care and case management services to patients receiving hospice care, ensuring comfort, dignity, and quality of life during their end-of-life journey. You will coordinate care plans, assess patient needs, and collaborate with interdisciplinary teams to deliver personalized support that aligns with patients’ wishes and medical requirements. Your expertise in acute and post-acute care settings will drive exceptional patient outcomes and foster meaningful connections with families and caregivers.
Responsibilities
Conduct thorough patient assessments, including vital signs, physiological evaluations, and symptom management to develop individualized care plans.
Manage patient intake processes, ensuring timely evaluation of eligibility and coordination of services.
Collaborate with physicians, social workers, chaplains, and other healthcare professionals to coordinate comprehensive hospice care.
Provide education to patients and families about disease progression, symptom control, medication management, and advance directives.
Monitor patient progress regularly through bedside visits and review of medical records to adjust care plans as needed.
Oversee discharge planning and post-acute care transitions to ensure seamless continuity of care across inpatient, outpatient, or home settings.
Maintain detailed documentation in compliance with HIPAA regulations and medical record standards while managing case files efficiently.
Ensure adherence to infection control protocols and safety standards within all patient interactions.
Support behavioral health needs by recognizing signs of emotional distress or mental health issues impacting patient well-being.
Qualifications
Valid RN license with current registration in the state of practice.
Demonstrated knowledge of hospital medicine, emergency medicine, infectious disease care, geriatrics, pediatrics (PICU), or behavioral health is advantageous.
Strong understanding of medical terminology, physiology, diagnostic evaluation procedures, and utilization management principles.
Proven ability in patient assessment, discharge planning, case management, and managing complex medical records.
Familiarity with HIPAA regulations and medical records documentation standards.
Excellent communication skills with a focus on patient service and compassionate care delivery.
Ability to coordinate multidisciplinary teams effectively while maintaining attention to detail in fast-paced settings such as emergency rooms or surgical units.
