Responsibilities
Deliver high-quality physical therapy sessions to patients on-site.
Develop personalized treatment plans aligned with patient goals and progress.
Ensure strict adherence to safety, ethical, and regulatory standards.
Maintain precise documentation of patient assessments, progress, and outcomes.
Monitor and report key performance indicators (KPIs) for patient outcomes and utilization.
Educate patients and family members on home exercises and self-management.
Collaborate with multidisciplinary team to optimize patient rehabilitation.
Identify opportunities to expand services and contribute to growth pipelines.
Demonstrate a growth mindset by seeking continuing education and skill development.
Participate in on-site performance reviews and contribute to merit-based rewards.
Assist in establishing treatment goals that align with territorial needs and market demand.
Experience/Qualifications
Licensed physical therapist with current state licensure and CPR/First Aid.
Deliver on-site PT sessions with evidence-based treatment plans.
Develop personalized treatment plans aligned to patient goals and progress.
Collaborate with multidisciplinary team to optimize rehabilitation plans.
Maintain accurate documentation of assessments, progress notes, and outcomes.
Monitor KPIs for patient outcomes, utilization, and service quality.
Educate patients and families on home exercises and self-management strategies.
Pursue continuing education; participate in mentorship and skill development.
Identify growth opportunities, contribute to service expansion pipelines.
Support performance reviews; pursue merit-based rewards through results-driven work.
Set treatment goals aligned with territorial needs and market demand.
Demonstrate strong accountability for personal performance in a team environment.
