St. Anthony Community Care Center, soon to be John A. Stassi II Community Care Center, will be the Greater New Orleans area’s newest skilled nursing & rehabilitation center, and we are looking to grow our team. Designed for person-centered care, John A. Stassi II Community Care Center will be the first of its kind in the area and will provide care in an atmosphere that promotes resident choice and creates a more home-like setting.
We are seeking the right candidate for the position of Clinical Care Coordinator RN. In this vital role, you will ensure that our residents receive the highest level of care while collaborating with a dedicated team to provide exceptional clinical leadership and support.
Primary Duties:
- Conduct comprehensive assessments of patients' medical records, including medical history, diagnoses, and treatment plans.
- Collaborate with interdisciplinary teams to gather necessary information for MDS assessments and Care Plans.
- Ensure compliance with federal and state regulations related to MDS assessments and Care Plans.
- Accurately code medical diagnoses and procedures using appropriate coding systems.
- Utilize clinical documentation improvement strategies to ensure accurate and complete documentation.
- Monitor and track patient progress and outcomes to support care planning and utilization management.
- Collaborate with healthcare providers as needed and oversee all clinical operations in assigned household.
- Maintain up-to-date knowledge of Medicare guidelines and regulations.
- Supervise nursing staff in assigned household and ensure all duties are being performed in accordance with expectations.
- Oversee Risk Management (i.e. falls, accidents, grievances, etc.) program for assigned household
- Lead daily huddles with household staff
- As part of the clinical administrative team, participate in on-call rotation
The Ideal Candidate:
- Previous experience in medical coding, MDS processes/planning, Care Plan management.
- Strong understanding of Medicare guidelines and regulations.
- Familiarity with utilization management processes.
- Experience with Matrix or similar electronic health record systems is a plus.
- Excellent attention to detail and organizational skills.
- Strong communication and interpersonal skills
- Organized and detailed in work performance.
- Excellent documentation skills; ability to write well to communicate patient care needs.
- Genuine care for and interest in residents of a nursing facility.
- Has not been excluded from participating in Medicare and Medicaid programs.
- Maintains eligibility to participate in Medicare and Medicaid programs throughout employment.
