Oversee all admissions from referral through admission
Coordinate with hospitals, case managers, discharge planners, and referral sources
Review clinical documentation to ensure appropriate placement
Manage insurance verification, authorizations, and payer requirements
Track referrals, conversion rates, and census trends
Work closely with nursing, social services, and administration to ensure seamless transitions of care
Serve as a point of contact for residents and families during the admissions process
Maintain compliance with state, federal, and facility admissions policies
