In this purpose-driven role, you will:
As the Admissions Coordinator, you will support the admissions process from referral through admission while helping ensure a smooth transition for residents and families entering the facility.
- Handle admissions inquiries, conduct tours, and assist with admissions paperwork
- Coordinate resident transitions from hospitals or home settings into the facility
- Communicate with hospitals, discharge planners, social workers, physicians, families, and referral sources
- Assist with census development and referral relationship activities
- Support implementation of facility marketing and community outreach efforts
- Maintain referral, demographic, financial, and waitlist information within EMR systems
- Assist with screening referrals utilizing clinical and financial guidelines
- Promote facility nursing, rehabilitation, and specialty services
- Ensure timely follow-up on referrals and admissions inquiries
- Assist with customer service initiatives and admissions-related follow-up
- Participate in daily huddles, admissions meetings, and assigned on-call rotations
Requirements:
- High School Diploma or Equivalent
