As a Care Coordinator you will provide case management/care coordination services to children and adolescents in the school/home/community and office environment who experience some type emotional disturbance. The CC works closely as a team with the Therapist, C&A In Home Team, family and other individuals involved in the provision of services to the consumer. The CC works with families to maintain appointments, monitor medications, and assist in coordinating outside resources such as Medicaid benefits, social security benefits, family food stamps, or securing other needed assistance. The CC also monitors behavior in various settings and report issues back to the therapist or In Home team as appropriate.
Qualifications
- Bachelor's degree in social work, psychology, or other human service related field is REQUIRED
- Experience in case management or care coordination, preferred.
- Knowledge of patient assessment, procurement of available resources and discharge planning preferred.
- Strong communication and interpersonal skills are necessary as well as self motivation.
- Ability to work effectively with children, adolescents, and their families are paramount in being successful in the position.
