Job Summary:

Responsibilities
- Engage with community members to assess their health needs and develop personalized care plans
- Provide education on topics such as nutrition, medication management, addiction counseling, and preventive health strategies
- Assist individuals with disabilities by coordinating services and facilitating access to healthcare resources
- Conduct motivational interviewing to encourage positive behavioral changes and enhance patient engagement
- Collect and document data related to health outcomes, service utilization, and social determinants of health for program evaluation
- Educate clients about Medicare benefits, medical terminology, and available community resources
- Advocate for clients within healthcare systems and connect them with appropriate social services and support networks
Requirements
- Certification as a Community Health Worker or equivalent credential from an accredited program
- Experience working with individuals with disabilities or in social work environments
- Knowledge of nursing practices, public health principles, and medical terminology
- Strong communication skills with the ability to speak publicly and deliver educational presentations
- Familiarity with data collection methods and maintaining accurate records
- Ability to apply motivational interviewing techniques effectively
- Background in teaching or health education is a plus; experience in addiction counseling or nutrition is desirable
