The Memory Care Coordinator supports not only clinical outcomes, but also emotional well-being, independence, and meaningful daily living-helping residents feel safe, valued, and truly at home. This position plays an essential role in supporting the staff working in The Woodlands and ensuring the residents in The Woodlands have purposeful connections.
This position is responsible for supporting the safety, well-being, and quality of life of memory care residents and for supporting care staff under the direct supervision of the Director of Nursing. The Memory Care Coordinator reports to the Director of Nursing and may be asked to perform other functions consistent with the employee's qualifications, training, certifications, and permitted scope of practice. Clinical or nursing duties may only be performed by an individual who holds the appropriate license, certification, training, and authorization. If the Memory Care Coordinator is not a licensed nurse, duties requiring nursing assessment, clinical judgment, medication administration, receipt or transcription of physician orders, or other licensed nursing functions remain the responsibility of the Director of Nursing or another appropriately licensed designee.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Resident Care:

Assist in answering call lights and pendant systems promptly and respond to resident needs with urgency and compassion.

Assist residents and staff with activities of daily living including eating, bathing, dressing, and mobility.

Ensure residents are being offered snacks and supplements as applicable, encourage hydration, and assist with feeding as trained and needed.

Perform care responsibilities that promote residents' safety, comfort, dignity, choice and independence.
-
Approach every interaction with patience, kindness, empathy, and respect.
-
Utilize person-centered and dementia-informed care techniques to support residents experiencing cognitive impairment, confusion, anxiety, or behavioral expressions, and coach staff as needed with oversight from the Director of Nursing.

Ensure the well-being, safety, and protection of residents by supporting the delivery of high-quality, person-centered care.
Medication AdministrationClinical Support:
- Promote a positive, collaborative work environment focused on teamwork and staff support.

Provide rotating on-call support for operational, staffing, and resident-service matters within the employee's qualifications and scope of responsibility; licensed nursing on-call needs are referred to the Director of Nursing or licensed nurse designee.

Assist the Director of Nursing with daily operational and regulatory compliance.

Provide direct caregiver or medication aide support when the department is short staffed, as applicable.

Provide services or support in Assisted Living, including during staffing shortages, as directed by the Director of Nursing or authorized designee.

Oversee and ensure safe, accurate, and timely medication delivery and administration following all policies and procedures.

Communicate any medication-related concerns, changes, or follow-ups to nursing leadership.

Coordinate with the Director of Nursing or authorized designee on mentoring staff, education, and other assigned duties within the Memory Care Coordinator's scope of practice and certifications.

Work with the Director of Nursing to collect resident information and contribute observations for annual care conferences, resident assessments, physician appointments, and other requested follow-up.

Assist the Director of Nursing with staff scheduling.

Support regulatory compliance efforts and participate in state licensure surveys.
Resident ExperienceEngagement:
- Help create and support daily opportunities for residents and team members to connect, contribute, and live with purpose through meaningful engagement.
- Assist with daily memory care programming.

Promote meaningful interactions during care-not just task completion.

Encourage resident participation in daily activities, dining, and social opportunities.

Observe resident mood, behavior, and engagement, recognize changes that may impact well-being, and promptly communicate resident changes in condition or well-being concerns to the Director of Nursing or another appropriately licensed designee.

Contribute to an environment where residents feel seen, heard, and valued.

Support the overall success of The Woodlands by monitoring resident and family satisfaction, staff performance, programming, environmental standards, occupancy readiness, and day-to-day operations, and by identifying opportunities for improvement in collaboration with the Director of Nursing or Executive Director.
DocumentationCommunication:

Accurately document outcomes related to resident care, medication administration, behavior, activity, and culinary services.

Communicate changes in condition, behavior, or care needs promptly to the Director of Nursing or authorized designee.
- Maintain clear, timely, and consistent communication with team members across shifts.
- Ensure all documentation meets regulatory and organizational standards.

- Work closely with nursing, care staff, culinary, and life enrichment teams to support the full resident experience.

Follow the company dress code accordingly as a Memory Care Coordinator and ensure staff dress code compliance in The Woodlands.
Safety, ComplianceInfection Control:
- Maintain confidentiality of all resident and staff health and personal information.
- Follow all Peaceful Pines policies and procedures, as well as State and Federal regulations.
- Adhere to OSHA and CDC guidelines for infection control and safety practices.
- Ensure care is delivered in a safe, clean, and supportive environment.
- Recognize and report safety concerns, incidents, or changes in condition promptly to the Director of Nursing or authorized designee.
GENERAL REQUIREMENTS:

Must submit to a TB test in accordance with state regulations.
- Must comply with all company policies, procedures, and safety standards.
- Must maintain confidentiality of resident and organizational information.
- Must be able to work flexible schedules, including weekends and holidays. Hours are determined by the needs of the Director of Nursing or authorized designee.

Must be able to respond effectively to resident needs in a timely manner.

Must demonstrate the ability to exercise sound judgment and promote a safe, comfortable, secure, and person-centered living environment for residents.
PREFERRED QUALIFICATIONS:
-
Active license in the state of South Dakota as a Medication Aide (or willingness to complete on-the-job training if not currently licensed).
- Current and unencumbered South Dakota RN or LPN license is preferred but not required. A non-licensed candidate must have qualifications and experience appropriate to the role and may not perform duties reserved for a licensed nurse.

Passion for working with seniors and/or caring for others.
- Strong interpersonal and communication skills.
- Dependable, team-oriented, and service-minded.
- Ability to provide care with patience, empathy, and attention to detail.
- A minimum of one-two years of experience in senior living, memory care, healthcare, or a related caregiving environment is preferred; experience supporting individuals living with dementia is strongly preferred.
