Position Overview
We are seeking a highly motivated Registered Dietitian and Certified Diabetes Educator to join our team. In this role, you will provide individualized nutrition counseling and comprehensive diabetes self-management education (DSME) to patients across a range of diagnoses, with a strong focus on metabolic health, weight management, and chronic disease prevention. This is an excellent opportunity for a clinician who is passionate about patient education and wants to be part of a collaborative, physician-led practice.
Key Responsibilities
- Conduct one-on-one nutrition assessments and develop individualized care plans for patients with diabetes, prediabetes, obesity, and related metabolic conditions
- Deliver diabetes self-management education and support (DSMES) in individual and group settings
- Collaborate with physicians and care teams to coordinate patient nutrition and diabetes management goals
- Monitor and document patient progress using the EMR system
- Provide Medical Nutrition Therapy (MNT) for a variety of conditions including diabetes, and weight management
- Support GLP-1 and metabolic health programming within the PRINE Wellness division
- Educate patients on lifestyle modifications including meal planning, carbohydrate counting, and behavior change strategies
- Maintain compliance with all clinical, HIPAA, and regulatory standards
- Participate in quality improvement initiatives and patient care meetings as needed
Qualifications
Required:
- Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential, current and in good standing
- Certified Diabetes Care and Education Specialist (CDCES) or Certified Diabetes Educator (CDE) credential, current and in good standing
- NYS Licensed Dietitian/Nutritionist (or eligible)
- Minimum 2 years of clinical experience in diabetes education or nutrition counseling
- Strong communication and patient education skills
- Proficiency with electronic medical records (EMR)
Preferred:
- Experience in an outpatient or multispecialty clinical setting
- Familiarity with GLP-1 therapy, metabolic health, or weight management programs
- Bilingual a plus
Job Type: Full-time
Pay: $36.00 - $41.00 per hour
