Job Summary
Direct clinical responsibility for evaluation and treatment of assigned patients. Participates in the Professional Advisory, Clinical Record Review and PI Committees. Provides physical therapy related education/support to staff and/or the community as requested. Evaluates the job performance of contract and/or other staff physical therapists. Makes recommendations to the Supervisor of Ancillary Services whenever corrective/disciplinary action to be taken. When assigned, performs duties of Clinical Supervisor of Ancillary Services, including assignment, scheduling and prioritization of rehabilitation cases.

1. Environmental Demands: Community driving and visiting patients I their homes, as well as normal hospital/office conditions. Moderate exposure to health hazards.

C. JOB SPECIFIC COMPETENCIES:
Demonstrates the required job specific technical/professional knowledge, skills and abilities. *
1. Provides physical therapy services: evaluation, direct patient care, teaching, supervision and consultation to patients in their homes, in accordance with state/federal regulations, professional and JCAHO standards.
2. Develops, reviews, implements and revises the patient Plan of Care in collaboration with the patient/family, and under the direction of the physician; initiates all necessary verbal and written physician orders pertaining to physical therapy services that are in addition to the original Plan of Care.
3. Coordinates and maintains effective communication in regard to patient services, with other involved disciplines: OT, ST, nursing, social work, home health aide. Reports patient findings to the physician as appropriate. Supervises home health aides in rehabilitation related duties including home exercise program.
4. For any assigned managed care patient, maintains contact with managed care case managers, obtaining all necessary authorizations for physical therapy services. Acts as case manager for assigned “Rehabilitation Only” patients.
5. Maintains comprehensive, accurate medical and other records, reports and communication in accordance with the policy of the Home Care department.

Qualifications
- Two years work experience as staff Physical Therapist and a minimum of one year in Home Care preferred.
- Physical Therapists - New York State Board of Physical Therapy
- BLS - American Heart Association
