Job Summary
We are seeking a dedicated and compassionate Care Manager to join our dynamic team. In this vital role, you will coordinate and oversee personalized care plans for individuals across diverse populations, including those with behavioral health needs, disabilities, or aging concerns. Duties
- Develop and implement individualized care plans tailored to each client’s unique needs, including behavioral health, addiction recovery, or disability support.
- Conduct thorough assessments to identify client needs related to crisis intervention, social work, child welfare, or workforce development initiatives.
- Coordinate services such as medical appointments, housing assistance (including Section 8), childcare, and Medicare-related support to promote stability and well-being.
- Utilize crisis management techniques and motivational interviewing to de-escalate situations and encourage positive behavior change.
- Provide supervision and mentoring to team members or interns involved in case management or social work activities.
- Engage in crisis intervention when urgent situations arise, ensuring immediate safety and connecting clients with emergency services as needed.
- Maintain detailed documentation of client interactions, service plans, progress notes, and incident reports in compliance with organizational standards.
Qualifications
- Proven experience in case management, social work, or behavioral health roles with a focus on crisis intervention and crisis management.
- Strong knowledge of addiction counseling, behavioral health strategies, and working with individuals with disabilities or geriatric patients.
- Certification or training in CPR and First Aid; experience with CPR is highly preferred.
- Demonstrated ability to work effectively with diverse populations including children, seniors, and individuals involved in child welfare or workforce development programs.
- Excellent communication skills with proficiency in motivational interviewing techniques; supervisory experience is a plus.
- Familiarity with healthcare programs such as Medicare and housing assistance programs like Section 8 is desirable.
- Ability to handle high-pressure situations confidently while maintaining a positive attitude; experience in mentoring or supervising staff is advantageous.
