Job Description: Perioperative RN – Pre/Post Op, PACU
Position Summary: All patients who will have been or have been subjected to general, regional block, spinal anesthesia, or local /w MAC will be cared for pre- operatively and postoperatively by a registered nurse who is qualified and competent to provide such care.
Accountability: The perioperative RN in the pre-op and PACU areas is directly accountable to the Director. This perioperative RN works in collaboration with Medical staff, nursing colleagues, PAs, certified nurse assistant, surgical technologists and Processing Tech. This perioperative RN directs LPNs and non-licensed staff.
Qualifications: 1. Current RN state licensure.
2. Prefer a minimum of 2 years of recent post-anesthesia, critical care or recovery care experience.
3. Current BCLS.
4. Current ACLS preferred or willingness to obtain.
5. Professional organization membership encouraged.
Physical
Requirements: The perioperative RN must be able to engage in physical activities on a daily basis. As direct patient care provider, the perioperative RN will maintain:
1. Hepatitis immunization (will be provided by BSA at no cost to the employee) or documented refusal to be immunized.
2. Evidence of negative PPD test or chest x-ray annually (will be provided at no cost to the employee).
3. Able to maintain extensive standing, moderate sitting, and assisting in the transfer of patients. Able to lift up to 40 lbs.
4. Physically able to perform CPR.
Responsibilities:
I. Assessment
The perioperative RN is accountable for the preoperative preparation, care and education of the patient. A pre-op telephone assessment will be conducted prior to the day of surgery. The telephone pre-op assessment will include pre-op and ASC protocols teaching. The perioperative RN is required to collect, interpret, record and communicate pertinent data that is relevant to the patient's care appropriate to pediatric, adolescent, adult and geriatric patients. Data is concurrently assessed for optimum patient care and planned discharge of the patient. Assessment shall include:
A. Verification of the patient's pre-operative vital signs and general conditions.
B. Verification of the operative procedure, site, surgeon, and type of anesthesia.
C. Utilizes all available resources in planning care and consults as necessary with resource persons. (anesthesia, surgeon, OR RN,
PA, ASC ancillary personnel).
D. Uses data collected to formulate nursing diagnosis about the health status and needs of the patient.
E. Revises plan of care as needed to accommodate patient's recovery based on continuing assessment of the patient.
F. Conveys the plan of care to other members of the surgical team.

II. Planning
The perioperative RN utilizes data collected to plan for pre-op and post-op nursing care appropriate to pediatric, adolescent, adult and geriatric patients
A. Follows the standards of care for post-op nursing care, along with the preferences of surgeons and anesthesia care providers, when planning the post-op care.
B. Monitors the necessary equipment and supplies for the Pre-op and PACU areas; equipment is checked and found to be in good working order.
C. Utilizes all available resources in planning care and consults as necessary with resource person (Physicians, Anesthesia, Clinical
Nurse Manager and other BSA team members).
D. Uses data collected to formulate nursing diagnosis about the health status and needs of the patient.
E. Revises plan of care as needed based on continuing assessment of the patient.
F. Conveys the plan of care to other members of the BSA team.
III. Implementation
The Preop Hold/PACU RN provides direct care to patients appropriate to pediatric, adolescent, adult and geriatric patients, meeting their physical and psychological needs:
A. Identifies priorities and executes nursing care accordingly.
B. Provides emotional support and comfort measures.
C. Gives information or explanation to patients and their families as warranted.
D. Ensures and respects the patient's rights, privacy and dignity.
E. Applies and monitors the principles of aseptic technique throughout the preoperative and postoperative phase.
F. May extubate the patient in accordance with established guidelines.
G. Maintains the intravenous infusion, may start new IVs as needed, discontinues IVs before discharge, records and delivers amount of solutions as ordered.
H. Administers medications and oxygen as needed, documents all administration.
I. Recognizes symptoms of cardiac arrest, respiratory distress, and/or other impending crisis; initiates emergency response.
J. Participates in the evaluation of new supplies and equipment and/or instruments.
K. Coordinates the activities of other health care professionals and ancillary staff in assisting the patient for discharge.
L. Instructs the patient in self-care and in home treatment of preoperative and post-operative symptoms.
M. Documents all preoperative and post-operative care.
N. Accurately and promptly documents unusual occurrences noted during the preoperative and post-operative phase of recovery.
O. Surveys patient record and recovery for QA as needed.
P. Completes post-discharge contact and call evaluations.
Q. Properly cares for and disinfects equipment and furniture in the recovery areas.
R. Reports and labels out of service equipment malfunction and documents return to service of equipment.
S. Participates in the evaluation of new supplies and equipment.

IV. Evaluation
A. The perioperative RN continuously evaluates the patient's response to nursing interventions and modifies the nursing care plan to achieve the desired outcome.
B. Collaborates with other disciplines to revise the patient care plan according to the changing needs of the patient appropriate to pediatrics, adolescence, adults and geriatric patients.
