What You’ll Do: Perform in-home skilled nursing visits, Complete comprehensive assessments and develop individualized care plans, Educate patients and families on disease management, Coordinate care with physicians and interdisciplinary team members, Complete accurate and timely documentation (including OASIS), Monitor patient progress and adjust care as needed

Job Summary
The Registered Nurse Case Manager plays a critical role in coordinating patient care across various healthcare settings. This position involves working closely with patients, families, and healthcare teams to ensure effective management of patient needs throughout their care continuum. The ideal candidate will possess strong clinical skills and experience in critical care, emergency medicine, and case management to facilitate optimal health outcomes.
Duties
- Conduct comprehensive patient assessments to determine individual needs and develop personalized care plans.
- Collaborate with multidisciplinary teams to coordinate and implement discharge planning and post-acute care strategies.
- Monitor patient progress and adjust care plans as necessary based on clinical evaluations and patient feedback.
- Provide education and support to patients and families regarding treatment options, health management, and resources available.
- Ensure compliance with HIPAA regulations while managing medical records and maintaining confidentiality of patient information.
- Utilize knowledge of medical terminology, physiology, and nursing practices to deliver high-quality patient service.
- Engage in utilization management to optimize resource use while ensuring quality care delivery.
- Participate in diagnostic evaluations and case management processes for various patient populations including pediatrics, geriatrics, and those requiring infectious disease care.
- Maintain up-to-date knowledge of best practices in acute care, outpatient services, clinic operations, and behavioral health.
Qualifications
- Current Registered Nurse (RN) license in the state of practice.
- At least 1 year of skilled Home Health experience is required
- Strong background in hospital medicine, medical-surgical nursing, or outpatient clinic experience preferred.
- Familiarity with case management principles including intake processes, discharge planning, and post-acute care coordination.
- Excellent communication skills with the ability to work collaboratively within a healthcare team.
- Knowledge of vital signs monitoring, patient assessment techniques, and managed care principles.
- Experience with electronic medical records systems is advantageous.
- Ability to handle complex situations with compassion while maintaining a focus on patient-centered care. This position requires a dedicated professional committed to enhancing the quality of life for patients through effective case management practices.
