Job Summary
We are seeking a dedicated and dynamic Registered Nurse (RN) to join our team in an innovative office and field role. This position offers a unique blend of clinical expertise and community engagement, allowing you to provide exceptional patient care both within our administrative office and in the field. As an RN in this role, you will be instrumental in coordinating patient assessments, managing care plans, and supporting post-acute and outpatient services. Your expertise will ensure seamless communication between patients, healthcare providers, and care teams, fostering positive health outcomes across diverse settings.
Duties
Responsibilities
Clinical Care & Patient Assessment
- Conduct comprehensive assessments of patients' physical and mental health status.
- Develop, implement, and evaluate individualized care plans based on patient needs and physician orders.
- Administer medications and treatments as prescribed, including injections, infusions, and other skilled nursing procedures.
- Monitor patient conditions, identify changes in health status, and communicate findings to the appropriate healthcare providers.
- Perform triage duties and respond appropriately in urgent or emergent situations.
Wound Care Management
- Assess, manage, and treat acute and chronic wounds using evidence-based practices.
- Perform wound assessments and develop individualized wound care plans.
- Apply wound care treatments, including dressing changes, debridement assistance, and negative pressure wound therapy (when appropriate).
- Monitor wound healing progress and document changes accurately.
- Educate patients and caregivers on wound care techniques, infection prevention, and skin integrity management.
- Participate in quality assurance and skin integrity initiatives while maintaining compliance with infection control standards.
Home Health & Field Responsibilities
- Conduct patient visits in home and community settings as assigned.
- Provide skilled nursing care while promoting patient independence and safety.
- Collaborate with physicians, therapists, caregivers, and other healthcare professionals to ensure continuity of care.
- Educate patients and families regarding disease management, medications, treatment plans, and preventive care strategies.
Documentation & Compliance
- Maintain accurate and timely documentation in the EMR system.
- Ensure compliance with all regulatory requirements, nursing standards, and agency policies.
- Document patient assessments, interventions, outcomes, and communications thoroughly and accurately.
- Stay current with nursing best practices and clinical advancements.
Qualifications Required
- Active and unrestricted Registered Nurse (RN) license.
- Minimum of 3 years of nursing experience.
- Strong assessment, critical thinking, and clinical decision-making skills.
- Excellent communication and documentation skills.
- Ability to work independently and manage multiple priorities.
- Reliable transportation and willingness to perform field visits.

Work Location
This is an on-site position that includes both office-based responsibilities and field/home visits.
Pay: $80,000.00 - $85,000.00 per year
