Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as POC.
- Administer medications and injections accurately following prescribed protocols, including IV infusions and medication administration for pain management or specialized treatments.
- Assist with medical procedures such as catheterization, venipuncture, tube feeding, and sterile processing techniques.
- Monitor patients’ conditions closely in acute care settings like providing critical interventions for trauma, infectious disease care, or post-operative recovery.
- Collaborate with physicians and interdisciplinary teams to develop and implement individualized care plans for patients with complex needs such as dementia, Alzheimer’s disease, or developmental disabilities.
- Manage patient documentation meticulously to ensure compliance with HIPAA regulations and accurate medical records updating.
Qualifications
- Valid Licensed Practical Nurse (LPN) license in good standing.
- Proven experience working in diverse healthcare environments such as hospitals (including ICU or trauma centers), nursing homes, hospice care, or outpatient clinics.
- Familiarity with EMR/EHR systems like Epic, POC or eClinicalWorks is highly preferred.
- Strong knowledge of physiology, anatomy, medical terminology, ICD coding (ICD-9/ICD-10), CPT coding, and infection control protocols.
- Experience with specialized areas including geriatrics, memory care (dementia/Alzheimer’s) is advantageous.
- Ability to perform clinical skills such as IV insertion/infusion therapy, phlebotomy, specimen collection/processing, airway management, and basic life support certification.
- Excellent patient assessment skills with a compassionate approach to working with individuals of all ages—including children with disabilities or those requiring assisted living services.
- Effective communication skills coupled with the ability to work efficiently within multidisciplinary teams in high-pressure environments like emergency rooms or Level II trauma centers.
