The Guided Assistance to Permanent Placement (GAPP) Program, SPIN’s HUD-funded Rapid Rehousing Program, has a long history of assisting households quickly transition out of homelessness and achieve sustainable housing. We are seeking a Senior Housing Case Manager to join our team and deliver high-quality, compliant, and outcome-focused services that make a meaningful impact in the community.
Position Summary
This position provides high-quality, housing-focused direct service and case management to ensure participants achieve and maintain long-term housing stability. This role manages a caseload of 10–20 households, blending intensive direct client support with rigorous data integrity in HMIS and full compliance with HUD Continuum of Care (CoC) requirements.
Key Responsibilities:
Case Management & Direct Service (Caseload: 10–20 participants)
· Provide intensive, housing-focused case management including assessment, housing stabilization planning, and crisis intervention.
· Support participants with housing search, landlord engagement, and connection to mainstream community resources.
· Conduct regular case management sessions and reassessments to promote long-term housing stability.
· Support daily program activities, ensuring services adhere strictly to Housing First and trauma-informed care principles.
· Maintain accurate, updated and secure client case files.
· Perform housing inspections and home visits, as required.
· Attend relevant meetings within the community, in person or online.
HUD Compliance & HMIS Administration
· Ensure accurate, timely, and complete data entry and reporting in the HMIS system.
· Maintain high standards of data quality through regular client file reviews and self-audits.
· Monitor participant files and documentation to ensure strict compliance with HUD regulations and agency policies.
· Assist the Program Manager with preparing documentation for funder reports, program audits, and monitoring activities.
Budget & Fiscal Coordination
· Assist with tracking program expenditures and ensuring the appropriate, compliant use of financial assistance funds for clients.
· Monitor rental assistance and supportive service expenditures for assigned households in alignment with grant requirements.
Qualifications
· Bachelor’s degree in social work, human services, psychology, or a related field preferred.
· 2+ years of experience in homeless services, housing programs, or intensive case management.
· Proven experience working with HUD CoC requirements, Rapid Rehousing programs, and HMIS systems.
· Experience working with diverse populations experiencing homelessness, including individuals and families with complex needs.
· Bilingual in English and Spanish
· Possession of superior interpersonal skills and a high level of emotional intelligence
· Knowledge of Homeless Management Information System (HMIS) or similar system

· Detail-oriented with strong compliance, file management, and documentation practices.
· Effective communication, conflict resolution, and partnership-building abilities.
· Ability to effectively manage multiple tasks and client priorities in a fast-paced environment.
Additional Requirements
· Background Check: Successful completion of a criminal background check.
· Driver’s License: Valid driver’s license and reliable transportation may be required for client meetings and community outreach.
