Position Summary
The Client Care Coordinator is responsible for overseeing the daily operations of caregiving services provided by Life Touch Health Personal Care at Harding Place. This role ensures high-quality care, staff supervision, scheduling coordination, and excellent customer service for residents, families, and facility partners. The Client Care Coordinator serves as the primary liaison between Life Touch Health Personal Care and Harding Place while maintaining a compassionate, professional, and resident-centered environment.
Key Responsibilities:
Staff Management & Supervision
- Ensure adequate staffing coverage for all assigned shifts.
- Conduct employee coaching, performance reviews, and corrective action when necessary (and with company director).
- Promote professionalism, teamwork, and compassionate care among staff.
Client Care Oversight
- Ensure clients receive quality personal care services according to individualized care plans.
- Monitor caregiver performance and client satisfaction.
- Address client, family, or facility concerns promptly and professionally.
- Maintain confidentiality and dignity of all clients.
Operations & Scheduling
- Oversee caregiver schedules, call-offs, and shift coverage.
- Coordinate intakes, service changes, and caregiver assignments.
- Maintain accurate documentation, incident reports, and service records.
- Ensure smooth daily operations within the facility.
Compliance & Safety
- Ensure compliance with state regulations, company policies, HIPAA guidelines, and facility standards.
- Monitor caregiving training and certifications if needed.
- Report incidents or concerns according to company and facility policy.
Communication & Relationship Building
- Serve as the main point of contact between the company and Harding Place.
- Build positive relationships with clients, families, staff, and healthcare professionals.
- Participate in staff meetings as needed.
- Support a positive reputation for the company within the community.
Requirements
- 40 hours a week (flexible daytime hours).
- Carry manager phone after hours.
Pay: $46,348.49 - $55,817.54 per year
