Overview
We are seeking a dedicated and detail-oriented MDS Coordinator - LVN (Licensed Vocational Nurse) to join our dynamic healthcare team. In this vital role, you will lead the comprehensive assessment and documentation process for residents, ensuring compliance with federal and state regulations, including Medicare and NCQA standards. Your expertise will help optimize patient care, facilitate accurate clinical documentation, and support effective care planning across inpatient, outpatient, and long-term care settings. Duties
- Conduct thorough Minimum Data Set (MDS) assessments for residents, ensuring accuracy and completeness in accordance with CMS guidelines.
- Review medical records, clinical documentation, and medical terminology to verify data integrity and compliance with HIPAA regulations.
- Collaborate with interdisciplinary teams—including nursing, case management, therapy, and physicians—to develop individualized care plans aligned with resident needs.
- Utilize EMR (Electronic Medical Record) systems such as Epic, Cerner, Athenahealth, or eClinicalWorks to document assessments and update patient records efficiently.
- Perform utilization review and management activities by analyzing clinical data to support appropriate resource allocation and discharge planning.
- Maintain expertise in ICD-10/ICD coding, CPT coding, DRG assignment, and case management procedures to ensure proper reimbursement and compliance.
- Participate in clinical documentation improvement initiatives to enhance the quality of medical records and support accreditation standards like NCQA.
Qualifications
- Valid LVN license with current state licensure required; additional certifications in managed care or utilization review preferred.
- Proven experience with MDS coordination in inpatient facilities such as hospitals or nursing homes; ICU or Level I trauma center experience is highly desirable.
- Strong knowledge of physiology, anatomy, medical terminology, and nursing practices related to acute care, hospice, emergency medicine, pediatrics, or primary care settings.
- Familiarity with EMR/EHR systems including Epic, Cerner, Athenahealth or similar platforms; experience with Meditech or eClinicalWorks is a plus.
- Experience working within managed care environments such as Medicare or Medicaid programs; understanding of HIPAA compliance is essential.
- Skilled in ICD-9/ICD-10 coding standards, CPT coding procedures, DRG assignment, and utilization management processes.
- Excellent documentation review skills coupled with attention to detail; ability to interpret complex medical records accurately.

Pay Rate is based on Experience
Pay: $35.00 - $45.00 per hour
