Job Summary
We are seeking a dedicated and compassionate In-Home Respite Case Manager to join our team, providing vital support to families and individuals requiring temporary relief through in-home respite services. This role involves coordinating care plans, assessing client needs, and ensuring the delivery of high-quality, patient-centered respite care. Position Summary
The In-Home Respite Case Manager is responsible for coordinating and managing individualized respite care plans for children and adults with I/DD and their families. This position acts as the primary point of contact for families, ensuring their needs are assessed, services are delivered as planned, and supports are culturally and linguistically appropriate.
Key Responsibilities
Family Engagement & Assessment
Conduct initial intake and needs assessments with individuals/families, including home visits as necessary.
Develop, review, and update individualized respite care plans in partnership with the family and clinical team.
Serve as an accessible, supportive resource to families throughout their participation in the program.
Service Coordination
Match families with appropriate DSPs; coordinate schedules and services based on family preferences and needs.
Arrange for emergency and crisis respite as needed, including after-hours response and coordination with community partners.
Ensure transportation, interpreter, and adaptive supports are in place for effective service delivery.
Monitoring & Documentation
Maintain accurate, up-to-date case records and service logs in the agency’s electronic case management system.
Track individual and family progress, satisfaction, and outcomes, adjusting service plans as needed.
Complete all required reports and participate in program evaluation and quality improvement activities.
Collaboration & Referral
§ Work collaboratively with other AHTIC staff, external providers, and community agencies to support holistic family needs (e.g., health, housing, benefits).
§ Refer families to additional community resources and supportive services (mental health, therapy, advocacy, or education) as appropriate.
Cultural Competence & Advocacy
Provide services that are sensitive to the cultural, linguistic, and socioeconomic backgrounds of families.
Advocate for the rights and needs of individuals with I/DD and their families within AHTIC and the broader service system.
Engage interpreters and cultural liaisons as needed to ensure full access to services and information.
Crisis Intervention
Respond to urgent family situations or crises (e.g., caregiver illness, hospitalization, abuse/neglect).
Coordinate with the Program Manager and DSP team to mobilize emergency respite and connect families to crisis resources.
Ensure all incidents are documented and reported in accordance with agency and state policies.
Qualifications
Bachelor’s degree in Social Work, Psychology, Special Education, Public Health, or related field required.
Minimum one year experience in case management, disability services, or family support (I/DD experience strongly preferred).
Demonstrated understanding of person-centered planning and community-based supports.
Bilingual or multilingual skills (preferred) and experience working with culturally diverse populations.
Strong organizational, interpersonal, and communication skills. Proficiency with electronic records and standard office technology.
